The 15-year-old died on April 8 after being injured during a match the Saturday prior playing in the Sawtell Junior Rugby League Club's under-16 team.

Among the mourners supporting his family were team mates, school friends and others from the Rugby League and Coffs Coast community including many of Jake's fellow players from other clubs in the region.

The grandson of NSW State of Origin Legend Tommy Raudonikis, Jake was remembered for his great talent on the football field and a star athlete who excelled at every sport he took part in.
